The following are step-by-step instructions on how to create an account and complete a
L.A. Found Program application for any of your referrals.
To register for a free L.A. Found account all participants, authorized representatives, caregivers, case managers/social workers, analysts, and program managers must create an account through L.A. Found.
1 – First, click on “Create an Account.”
2 – Enter your desired username. All usernames must have a minimum of 9 characters, at least one letter and one number. Repeated characters are not accepted.
3 – Enter the first and last name of the individual creating the account.
4 – Enter a valid email address. This is where you will receive all application electronic communication.
5 – Enter your desired password and confirm your selection.
6 – For the final step, you will be required to confirm your account. Check your email and click on the link to finalize the registration process.
7 – Once you have confirmed your account, you can begin the online application process.
– For the purpose of this application authorized agent is the person who is dully responsible and legally capable of placing a tracking device on the participant.
– For the purpose of this application the caregiver is the person who is responsible for caring for the participant in the case the authorized representative is not available.
– For the purpose of this application the participant is the person who is at risk of wondering and getting lost.
– When logged into created account, all authorized representatives, caregivers, or case managers/social workers applying for a L.A. Found device for a participant must apply through the new application link located on the top left-hand side under dashboard link.
1. Participant Information Page:
Participant information page has a purpose of obtaining needed information about the person wearing the device with the purpose of assisting authorized agents and caregivers in the case of an emergency. Please provide information on all required fields indicated with a red asterisk (*).
a. The following fields are mandatory and required to complete the participant information page: Interested in purchasing or a free device, First Name, Last Name, Date of Birth, Address, Zip Code, Home phone, Is participant a Veteran, Language, Does participant require a translator?, Does responsible party require a translator?, Cognitive Impairments, Mobility Impairments, Difficulty Communicating
This section provides all personal identifiable information for the participant that will be using the device. All information request is pertaining to participant. Although, some information is not required to proceed, we encourage to provide as much information as possible for a timely processing of the application.
2. Authorized Representative and Caregiver Information Page:
Authorized representative page has a purpose of obtaining needed information about representative authorized to place a tracking device on a participant for the purpose of locating an individual in case of an emergency. Caregiver information page has a purpose of obtaining needed information about the caregiver responsible in caring for the program participant. Please note if caregiver and authorized agent are the same individuals, we have provided a box to be checked and all information will be automatically generated. Please provide information on all required fields indicated with a red asterisk (*).
a. The following fields are mandatory and required to complete the authorized agent information page: First Name, Last Name, Address, Zip Code, Home phone, Relationship, How did you hear about us? Please note if participant and authorized agent share the same home address, we have provided a box to be checked and all information will be automatically generated. Although, some information is not required to proceed, we encourage to provide as much information as possible for a timely processing of the application.
3. Attachment Page:
Attachment page has a purpose of allowing authorized agents to upload required documentation to complete application process. Please note that PDF and JPEG are the required document files types permitted for uploading.
a. The following documents are required to be uploaded to complete the application process. It is important that a clear picture or image is uploaded in order not to delay the application process.
1 – Copy of ID for person who will be wearing device (participant), in the event an ID is not available for a minor please provide a birth certificate.
2 – Copy of ID for participant’s Authorized Agent, an authorized agent is an individual with a Power of Attorney, Conservatorship, Guardianship, or is authorized to make medical decisions for the person in an advance Health Care Directive.
3 – Copy of documentation verifying Authorized Agent status (i.e., Power of Attorney, Conservatorship documents, Guardianship or Advance Health Care Directive).
4 – Copy of a full body picture for the person that will be wearing the device (participant).
b. Documents may be denied if illegible or incorrect documentation is provided.
4. Acknowledgment Page:
Please read the disclaimers on the acknowledgement page. By checking the boxes, you release L.A. County, L.A. Found Program, L.A. Sheriffs, and all associated agencies and departments from liability in the case that the program does not have the intended outcome. L.A. Found does not guarantee that every participant will be located. Various issues and or human error can possibly alter the outcome of every search. This page also outlines all responsibilities for all parties involved. Agreement is required to complete the application process.
a. The following areas are required for completion of the acknowledgment page and must be completed prior to system allowing application to be submitted.
1 – Agree with terms and conditions.
2 – Certify that all information is accurate to the best of knowledge.
3 – Program contract and release section including agents and participants names.
4 – Provide or deny LA County with permission to use photographs for the purpose of L.A. Found initiative.
5 – Affirm that Authorized Agent has understood all contracts, release of waivers and liabilities.
6 – Confirm basic authority as an agent and electronically sign acknowledgment page.
b. Upon submission of completed application, application will be reviewed in its entirely. This includes all documents uploaded, content for participant, authorized agent, and caregiver, and all signatures.
5. Referral Page:
The referral form allows community partners to provide a warm hand off to the L.A. Found Program and assist in the beginning of the application. L.A. Found staff will call referred applicants and help them complete the application.
a. Referral Processing, L.A. Found Team will:
1 – L.A. Found team will – follow up with every referral form completed by its partners and provide program guidelines to all referrals.
2 – L.A. Found team will – assist participants and families with the completion of a program application.
3 – L.A. Found team will document every contact with referral and its outcome.
b. Monthly Reporting to LADOA:
1 – L.A. Found will provide monthly reporting to City of Los Angeles Department of Aging (LADOA) of all referrals sent by its partners.
