The Supportive Services Program (SSP) helps older adults maintain independence, improve daily functioning, and remain safely in their homes and communities.
The program provides a range of in-home and community-based services that support health, reduce isolation, and help individuals continue living with dignity.
You may qualify if you:
• Are age 60 or older.
• Live in Los Angeles County (excluding the City of Los Angeles).
• Have functional limitations or need assistance to remain independent.
• Priority is given to individuals who are:
• Frail.
• Homebound due to illness or disability.
• Socially isolated.
The following service categories are provided under SSP.
• Comprehensive assessment of physical, psychological, and social needs.
• Development of a written care plan.
• Coordination of services and ongoing follow-up.
• Assistance with daily physical needs.
• Support may include supervision, walking, and grooming.
• Help with meal preparation and light housekeeping.
• Assistance with shopping and basic household tasks.
• Support with tasks such as phone use and managing daily needs.
• Temporary relief for caregivers.
• Helps support caregivers of older adults living at home.
• Services provided at licensed adult day care facilities.
• Specialized support for individuals with Alzheimer’s disease or other forms of dementia.
• Matching individuals with screened, self-employed workers.
• Helps connect participants with in-home assistance providers.

Yes. A case manager conducts an in-home assessment.
Approval is immediate once eligibility is verified and consent for services is provided.
Services are generally provided for about six months, but this may vary depending on individual needs.
• Follow-up occurs at least every three months.
• Reassessments are conducted every six months.
• A new assessment is conducted at the start of each fiscal year for continued services.
You may request a preferred provider. If there is a waitlist, providers prioritize individuals within their service area.
Yes. Providers serve specific geographic areas and may prioritize clients within those areas.
SSP may supplement other services but cannot duplicate them.
SSP services cannot duplicate services you are already receiving.
