Prevention and Health Promotion (DPHP)

About the Program

The Disease Prevention and Health Promotion (DPHP) program helps older adults stay healthy, active, and independent. It offers free workshops that teach practical skills to prevent illness, manage ongoing health conditions, reduce falls, and improve overall well-being.

All workshops are evidence-based, meaning they have been tested and proven to improve health outcomes. Most are offered in small group settings, either in person or online.

Who It’s For?

You may qualify if you:

• Are age 60 or older, or an adult (18+) living with a disability

• Live in Los Angeles County (outside the City of Los Angeles)

Program Details

DPHP offers a variety of structured group workshops designed to support your health and well-being.

The Following Services Categories Are Provided Under DPHP:

Chronic Disease & Health Management

• Chronic Disease Self-Management Program (including Spanish: Tomando Control de Su Salud)

• Diabetes Self-Management Program (including Spanish-language version)

• Chronic Pain Self-Management Program

Fall Prevention & Safety

• A Matter of Balance (fall prevention and confidence building)

• Healthy Steps for Older Adults (fall risk awareness and prevention)

Physical Activity & Fitness

• EnhanceFitness (strength, balance, and flexibility)

• Arthritis Foundation Exercise Program

• Walk With Ease (arthritis-friendly walking program)

• Tai Chi for Arthritis (balance and mobility)

Wellness & Education

• Bingocize (combines games with movement and health education)

• WISE (Wellness Initiative for Senior Education)

• Medication Safety

• HomeMeds (helps identify and reduce medication-related risks for eligible participants)

What to Expect

• Workshops are free of charge.

• Programs are offered in small group settings.

• Sessions are led by trained facilitators.

• Classes may be held at senior centers, community sites, residential facilities, or online.

• You can enroll in more than one program and repeat programs if helpful.

• Some classes may have limited space, so pre-enrollment is required.

Common Questions

Each workshop runs for a set number of sessions over several weeks. Examples include:

• 6-week programs (about 2–2.5 hours per session)

• 8–16 week programs (about 1–2 hours per session)

• Some fitness programs may run longer (up to 24 sessions)

• Exact schedules vary by program.

Classes are typically offered at:

• Senior centers

• Parks and recreation facilities

• Senior housing communities

• Online (for select programs)

Workshops are offered in scheduled series (cycles) throughout the year, depending on community demand and location availability.

Yes. Pre-enrollment is required because class sizes are limited.

Mobility accommodations: Yes

Vision/hearing accommodations: Available in some cases (may be limited)

Class sizes are kept small to support participation and interaction.

Workshops are led by trained facilitators through Partners in Care Foundation.
