The Linkages Program provides care management services that connect older adults and adults with disabilities to a range of services and supports.
The goal of the program is to help individuals remain safely in their homes and communities and prevent or delay placement in a nursing home or other institutional setting.
The program also supports individuals who are preparing to return to the community after a stay in a residential care facility.
You may qualify if you:
• Are age 18 or older and live in Los Angeles County.
• Are at risk of entering a nursing home or institutional setting.
• Are able to live safely in the community with support.
• Need case management services and are willing to participate.
• Are not receiving other case management services.
The following service categories are provided under Linkages.
• Intake screening and in-home assessment.
• Development of a care plan based on individual needs.
• Ongoing monitoring, follow-up, and reassessment.
• Coordination with family members, friends, or other informal supports.
• Ongoing communication through phone calls or home visits.
• Coordination with community and publicly funded services.
• Connection to services that support the care plan.
• Services provided when needs cannot be met through other resources.
• Services are approved based on individual need and program guidelines.

Care management is ongoing, with yearly reevaluation of the care plan.
• A referral is sent to the Aging & Disabilities Department’s AAA team.
• The referral is forwarded to a provider.
• The provider will assess your needs and coordinate services.
• You may be considered at risk if you:
• Have difficulty with one or more activities of daily living (ADLs).
• Have difficulty with two or more instrumental activities of daily living (IADLs).
• Are unable to manage your own affairs due to emotional or cognitive concerns.
• Have experienced a significant event or change within the past 12 months.
No. You cannot receive Linkages if you are already receiving other case management services.
Purchased services:
• Must address needs that cannot be met through other resources.
• Must support reducing the risk of institutionalization.
• Are available only to individuals receiving care management services.
Services may end if:
• Care management goals are met and services are no longer needed.
• Your condition improves or stabilizes.
• You require a higher level of care.
• You move out of the service area.
• You choose to leave the program.
• You are no longer able or willing to participate.
